First Oral Surgery Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Your medical history will be reviewed and your anesthesia options will be presented. Review of your panoramic x-ray and/or 3D CAT Scan will aid in the development of a precise treatment plan. When your treatment plan is complete, the business team will verify your insurance, initiate any prior authorizations needed, and give you an estimate of the financial responsibility you will have on the day of your surgery.

Please be sure to have the following information when you come to your appointment:

  • Your surgical referral slip and your panoramic x-ray if one has been taken.
  • A list of any medications you are currently taking.
  • If you have medical or dental insurance, bring appropriate insurance cards for each. If you do not have the necessary insurance information, we will be unable to bill the insurance for your visit and you will be responsible for payment in full.

  • IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at both the consultation and surgery appointments.

A preoperative consultation is mandatory for patients undergoing IV anesthesia for surgery. Verbal and written pre-operative anesthesia instructions will be given at the consultation. (also see “Pre-Op Anesthesia Instructions” in the Surgical Instructions section our website.)

Please alert us during your consultation or prior to your surgery if you have a medical condition that may be of concern (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, osteoporosis/osteopenia, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist has taken a panoramic x-ray, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If no panoramic x-ray has been taken,it can be taken at our facility. In addition to our digital panoramic x-ray machine, we are excited to announce the addition of a 3D CAT Scan to our practice.

First Cosmetic Services Visit

For your first visit on the way to becoming a more youthful looking you, please bring any pictures of yourself that will give Dr. Smith an idea of what look you are hoping to accomplish with the treatments he will provide.  Also bring a list of any medications that you are currently taking.

Please be aware that Cosmetic Services are not covered by insurance and, therefore, payment for the Cosmetics Consult will be due on the day of the consult. 

During the consult, Dr. Smith will determine which product to use to give you as well as the amount of product needed to provide you with the absolute best results.  A treatment plan for the services will be made.  That treatment plan will also inform you as to the cost of the treatments you desire.  In order to make your appointment for the actual services, a 50% deposit will need to be made.  The remaining 50% will be due on the day your services are completed.
